ACT 453 (SB 732) by Sen. Hebert and Rep. Kleckley from the 2010 Legislative Session, requires all noncontracted, facility-based physicians to display specific information related to balance billing on patient bills,  effective July 1, 2011.

As outlined in Section D of the Act, billing statements for out-of-network
patients must include:

1. An itemized list of the services and supplies provided along with the dates such services and supplies were provided.
2. The amount that is owed by the patient prominently displayed on the front of the bill: “NOTICE: THIS IS A BILL. BASED UPON INFORMATION FROM YOUR HEALTH PLAN, YOU OWE THE AMOUNT SHOWN.”
3. A telephone number to call to discuss the statement.
