Pre-Application

Application for Continued Accreditation

Variable                  Site-Survey Expense Reimbursement
***The Application Fee is due upon submission of the Application. 
***The full Maintenance Fee is determined according to the number of years of accreditation awarded and is payable at the time accreditation is awarded.  Fees for a four year accreditation would be $6500 plus any Site-Survey expenses
 
 
Annual Provider Fee
$250                        January 31, 2011
$450                        January 31, 2012
$550                        January 31, 2013 and every year thereafter
***Annual Provider Fees are due each January 31st with Provider Annual Reports 
 
 
Late Fees  
$350                     
***Providers with reports or Applications not received by the deadline will be invoiced a $350 Late Fee
 
 
Other Penalties
Fees not paid within 30 days of invoicing will result in a penalty of an additional 10% of the total amount due.  Fees not paid within 90 days of invoicing will result in the lapse of the organization's accreditation.
